Yeah you need to buy a PSN card with yen on it, the best place for that is
play-asia.com; you can either get the cards sent to you physically or the codes e-mailed to you almost immediately, though I really wouldn't see the point in having it mailed to you unless its some kinda of limited edition card with a nice design. As far as making the account, the only thing really made up is country of residence, prefecture, street address, and zip code. The e-mail needs to be real and not the same as the e-mail you used you your US PSN account. I guess you could lie about age, gender, name, etc. if you wanted to, but you don't have to.

According to what I could find the easiest way to redeem a JP PSN card is to go to psn>account management>second option while signed into your japanese account. Although I know theres a way to do it from in the PSN store too. (which is probably super easy if you actually know Japanese, but I don't)
